Apollo 16 Lunar Module Data Checklist Page – Training Used – Signed & Certified by Charlie Duke

A rare and authentic Apollo-era training artifact: an original Lunar Module Data checklist page used during Apollo 16 mission training, hand-signed and certified by Charles Duke, Lunar Module Pilot of the mission.

During the Apollo 16, astronauts relied on compact LM Data checklist books to guide them through complex spacecraft procedures. These cards contained step-by-step operational instructions used during simulations and actual mission phases aboard the Apollo Lunar Module Orion.

Training copies like this example were used extensively in mission simulations, where astronauts rehearsed every stage of the lunar landing sequence—from descent preparations to surface operations and ascent from the Moon. The notes, layout, and format of these checklists reflect the highly structured procedures that ensured safe lunar exploration during the Apollo program.

Apollo 16 launched in April 1972 with Commander John Young, Lunar Module Pilot Charlie Duke, and Command Module Pilot Thomas Mattingly. Duke and Young landed in the Descartes Highlands, conducting three lunar surface excursions and collecting valuable geological samples that expanded scientific understanding of the Moon.

This large 8 × 10 inch checklist page represents a working component of the documentation astronauts used while preparing for the mission—making it a tangible link to the training that made lunar exploration possible.
