Lunar surface flown 8x10 inch sized checklist "ECS Malfunction" - pages 70A+70B from the lunar surface flown "Apollo 15 LM Malfunction procedures Manual". 

This checklist stayed on the lunar surface inside the Lunar Module for a total of three days during the first extended scientific exploration of the moon at Hadley Apennine. 

In addition to our own COA a notarized COA has been done here in Germany that comes with the checklist together with a full report (translated into English from German) by the public notary.

We have obtained the entire Malfunction checklist originally sold by astronaut David Scott in auction.
